Perimenopause: The 10-Year Transition Nobody Talks About
What Is Perimenopause?
The transitional period before menopause when ovarian function becomes erratic. Lasts 4-10 years, beginning as early as the mid-30s. Average age of menopause is 51.
Thank you for reading this post, don’t forget to subscribe!- Early perimenopause: Cycles shorten to 24-25 days. Progesterone declining. Subtle symptoms.
- Late perimenopause: Highly irregular cycles. Estrogen wildly fluctuating. Classic symptoms peak.
- Menopause: 12 consecutive months without a period.
The Full Range of Symptoms
- Vasomotor: Hot flashes, night sweats
- Sleep: Insomnia, poor quality
- Cognitive: Brain fog, word-finding problems, memory lapses
- Mood: New-onset anxiety, irritability, depression — the highest lifetime risk of new-onset depression
- Genitourinary: Vaginal dryness, painful intercourse, recurrent UTIs
- Physical: Joint pain, heart palpitations, abdominal weight gain, hair thinning
Why It’s Often Missed
- Only 20% of OB/GYN residencies include menopause curriculum
- Lab tests may be normal one day and menopausal-range the next
- Perimenopause is a clinical diagnosis based on symptoms and age — not lab values
- Anxiety and brain fog frequently misdiagnosed as GAD or depression
Evidence-Based Treatments
Hormone Therapy (HT)
For women within 10 years of menopause and under 60, benefits generally outweigh risks. Transdermal estradiol (patch, gel) avoids clotting risk of oral estrogen. Micronized progesterone has more favorable profile than synthetic progestins.
Non-Hormonal Options
SSRIs/SNRIs (low-dose paroxetine, venlafaxine), gabapentin, fezolinetant (Veozah, FDA-approved 2023 — novel NK3 receptor antagonist).
Lifestyle
Resistance training (critical for bone density), CBT for hot flashes, soy isoflavones (modest effect), adequate calcium and vitamin D.
Seek a NAMS-certified menopause practitioner or a specialized telemedicine platform (Midi Health, Evernow).
